SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join the team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face. This includes assisting with adjustments to institutional living, financial concerns, family relationships, medical issues, aging-related changes, discharge planning, and counseling for end-of-life care.

Responsibilities

• Provide clinical social work services to patients and families.
• Assess and address common psychosocial issues.
• Collaborate with medical teams to provide holistic care.
• Offer counseling and support for end-of-life care.

Requirements

• Bachelor's in Social Work (BSW) required, Master's degree preferred.
• Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
