SOCIAL WORKER

Job Description

We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges they may face. This role involves helping individuals adapt to institutional living, manage financial concerns, navigate family relationships, and provide support in difficult situations such as death and dying.

Responsibilities

• Provide clinical social work services and psychosocial assessments to patients and families.
• Address common issues including adjustment to institutional living, financial concerns, family relationships, aging, and discharge planning.
• Offer counseling related to medical diagnoses, treatments, and end-of-life care.

Requirements

• Bachelor's in Social Work (BSW) required, Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification.
