Job Description
We are looking for a compassionate and dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common issues such as adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Conduct psychosocial assessments of patients/residents and families.
- Provide counseling and support regarding various challenges related to medical conditions, aging, and discharge planning.
- Collaborate with interdisciplinary teams to ensure comprehensive care for patients.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred; LGSW certification required.
- Previous experience in a healthcare setting is advantageous.
- Strong communication and interpersonal skills.
