Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services to patients/residents and families.
- Address issues related to adjustment to institutional living, financial concerns, and family relationships.
- Offer counseling related to death and dying.
- Participate in discharge planning.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C preferred.
- LGSW certification required.
