Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health. As part of our team, you will provide clinical social work services and psychosocial assessments to patients, addressing a range of common issues such as adjustment to institutional living, financial concerns, and family relationships.
Responsibilities
- Conduct psychosocial assessments for patients and their families.
- Provide counseling and support related to various challenges including aging, discharge planning, and end-of-life care.
- Collaborate with the healthcare team to ensure comprehensive care for patients.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW).
- Prior experience in a clinical setting.
