Job Description

We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide essential clinical social work services and psychosocial assessments to patients, addressing various challenges they may face. This full-time position at Levindale Hospital in Baltimore, MD, offers a day shift schedule from 8:00am to 4:30pm.

Responsibilities

• Offer clinical social work services and psychosocial assessments to patients and their families.
• Address issues such as adjustment to institutional living, financial concerns, family relationships, and end-of-life counseling.
• Collaborate with healthcare teams to support patients in various stages of their healthcare journey.
• Facilitate discharge planning and provide counseling on challenging life transitions.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree in Social Work preferred.
• Licensed Certified Social Worker - Clinical (LCSW-C) preferred.
• Licensed Graduate Social Worker (LGSW) certification.
