Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, focusing on various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life care.
Responsibilities
- Conduct psychosocial assessments for patients and families.
- Provide counseling and support for patients facing medical challenges.
- Assist with discharge planning and end-of-life care.
- Collaborate with interdisciplinary teams to address patient needs.
Requirements
- Bachelor's degree in Social Work (BSW) required; Master's degree preferred.
- Licensed Certified Social Worker-Clinical (LCSW-C) preferred.
- Licensed Graduate Social Worker (LGSW) certification.
- Strong communication and interpersonal skills.
