Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common concerns such as adjustment to institutional living, financial issues, family relationships, and more.
Responsibilities
- Provide clinical social work services and psychosocial assessments.
- Address common issues related to patients' well-being and adjustment.
- Work closely with patients/residents and families to offer support and counseling.
- Participate in discharge planning and end-of-life counseling.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification required.
- Strong interpersonal and communication skills.
- Compassionate and empathetic approach.
