Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health at Levindale Hospital in Baltimore, MD on a full-time basis. As a member of our team, you will provide clinical social work services and psychosocial assessments to patients/residents and their families, addressing a range of common challenges.
Responsibilities
- Offer support and counseling to individuals and families facing various difficulties.
- Assess and address issues related to institutional living, financial concerns, family dynamics, aging, and end-of-life planning.
- Collaborate with healthcare professionals to ensure holistic patient care.
- Participate in discharge planning and assist in transitioning patients to appropriate care settings.
Requirements
- Bachelor's in Social Work (BSW) required, Master's degree preferred.
- LCSW-C certification is preferred.
- LGSW certification is required.
- Strong communication and interpersonal skills.
