Job Description
We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a key member of our healthcare facility, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing a range of common challenges such as adjustment to institutional living, financial concerns, and counseling related to end-of-life issues.
Responsibilities
- Conduct psychosocial assessments and provide counseling to patients and families.
- Assist with discharge planning and support patient transitions.
- Collaborate with healthcare teams to ensure comprehensive care for patients.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
- Strong communication and interpersonal skills.
