We are looking for a compassionate and dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As a full-time Social Worker, you will be responsible for providing clinical social work services to patients and their families, addressing various psychosocial needs and challenges. Responsibilities

• Conduct psychosocial assessments and provide counseling to patients and families.
• Assist patients with adjustment to institutional living and aging-related issues.
• Facilitate discharge planning and support patients in end-of-life care.
• Collaborate with multidisciplinary teams to ensure holistic patient care.

Requirements

• Bachelor's in Social Work (BSW) required, Master's degree preferred.
• Licensed Certified Social Worker-Clinical (LCSW-C) certification preferred.
• Licensed Graduate Social Worker (LGSW) certification required.
• Excellent communication and interpersonal skills.
