Job Description
We are seeking a dedicated Social Worker to join LifeBridge Health in Baltimore, MD. As part of the team at Levindale Hospital, you will provide essential clinical social work services and psychosocial assessments to patients and their families, addressing various challenges such as institutional living, financial concerns, family dynamics, medical understanding, aging issues, discharge planning, and end-of-life counseling.
Responsibilities
- Offer clinical social work services and psychosocial assessments.
- Address patient and family needs related to institutional living and medical procedures.
- Assist with discharge planning and end-of-life counseling.
- Collaborate with healthcare teams to provide comprehensive care.
Requirements
- Bachelor's in Social Work (BSW) required; Master's degree preferred.
- LCSW-C preferred; LGSW certification.
- Strong communication and interpersonal skills.
- Ability to work in a fast-paced healthcare environment.
