Job Description
We are looking for a dedicated Social Worker to join our team at LifeBridge Health. This full-time position, based in Baltimore, MD at Levindale Hospital, offers a day shift schedule. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing various common issues such as adjustment to institutional living, financial concerns, family relationships, and more.
Responsibilities
- Conduct clinical social work services and psychosocial assessments.
- Support patients and families with adjustment, financial, and family-related issues.
- Provide counseling related to medical procedures, aging, and end-of-life planning.
- Collaborate with the healthcare team for discharge planning.
Requirements
- Bachelor's in Social Work (BSW), Master's degree preferred.
- LCSW-C preferred, LGSW certification.
- Prior experience in a healthcare setting is a plus.
