SOCIAL WORKER

Job Description

We are looking for a compassionate Social Worker to join our team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients, residents, and families, addressing various challenges such as adjustment to institutional living, financial concerns, family relationships, and end-of-life planning. This role plays a vital part in supporting individuals during challenging times and making a positive impact on their lives.

Responsibilities

• Conduct psychosocial assessments and provide counseling to address emotional, social, and psychological needs.
• Collaborate with healthcare teams to develop and implement care plans.
• Assist patients and families in navigating healthcare systems and community resources.
• Participate in discharge planning and support patients in adjusting to changes relating to aging.

Requirements

• Bachelor's degree in Social Work (BSW); Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification.
• Excellent communication and interpersonal skills.
