SOCIAL WORKER

Job Description

We are looking for a dedicated Social Worker to join our compassionate team at LifeBridge Health. As a Social Worker, you will provide clinical social work services and psychosocial assessments to patients/residents and families, addressing a variety of common challenges and concerns. This role involves supporting individuals in adjustment to institutional living, financial issues, family dynamics, medical understanding, aging-related changes, discharge planning, and counseling for end-of-life matters.

Responsibilities

• Provide clinical social work services and psychosocial assessments.
• Assist patients/residents and families in coping with various challenges.
• Engage in discharge planning and counseling for end-of-life issues.
• Collaborate with the healthcare team to address patient needs effectively.

Requirements

• Bachelor's in Social Work (BSW) required; Master's degree preferred.
• LCSW-C certification preferred.
• LGSW certification.
• Compassionate attitude and strong interpersonal skills.
