Job Description
We are looking for a compassionate Social Worker to join LifeBridge Health in Baltimore, MD. As a key member of our team, you will provide clinical social work services and psychosocial assessments to patients and their families, addressing various challenges, including adjustment to institutional living, financial concerns, and counseling related to death and dying.
Responsibilities
- Provide clinical social work services to patients and families.
- Conduct psychosocial assessments and address common social problems.
- Assist with discharge planning and counseling related to aging and death.
Requirements
- Bachelor's in Social Work (BSW); Master's degree preferred.
- LCSW-C preferred.
- LGSW certification.
