Job Description
We are looking for a dedicated Social Worker to join the inpatient psychiatric team at Northwest Hospital. This part-time position offers day shifts on weekdays, providing critical coordination of psychosocial assessment and discharge planning for patients. As a key resource, you will utilize your expertise to support patients' needs, ensure quality care, and facilitate smooth transitions through the continuum of care.
Responsibilities
- Act as a coordinator of psychosocial assessment and discharge planning.
- Provide leadership and psychosocial support to patients receiving inpatient psychiatric services.
- Utilize knowledge of community and health care resources to address individual needs.
- Facilitate quality resource utilization to optimize patient outcomes.
Requirements
- Master's degree in social work, counseling, or related field.
- Maryland Social Work licensure required (LMSW required, LCSW-C preferred).
- Prior experience in social work.
