SOCIAL WORKER-I/P PSYCH-LCSW

Job Description

We are looking for a dedicated Social Worker-I/P Psych-LCSW to join our team at LifeBridge Health. As a key member of our inpatient psychiatric services at Northwest Hospital in Randallstown, MD, you will play a vital role in coordinating psychosocial assessment and discharge planning for our patients. Your leadership and support will be crucial in addressing the individual's needs and optimizing resource utilization for quality care.

Responsibilities

• Coordinate psychosocial assessment and discharge planning for inpatient psychiatric patients.
• Provide leadership and psychosocial support to address patient/client needs pre-admission to post-discharge.
• Utilize knowledge of community and healthcare resources to assist in meeting identified needs.
• Facilitate quality resource utilization by intervening in a timely and appropriate manner.

Qualifications

• Master's degree in social work, psychology, or related field.
• 3-5 years of relevant experience preferred.
• Maryland Licensed LCSW; LMSW candidates will also be considered.
