Job Description

We are looking for a dedicated Social Worker I/P Psych-LCSW to join our team at LifeBridge Health. In this role, you will serve as a pivotal coordinator for psychosocial assessment and discharge planning for inpatient psychiatric patients, offering essential support and resources. Your expertise will assist in optimizing resource usage and care quality before and after hospitalization.

Responsibilities

• Coordinate psychosocial assessments and discharge planning for inpatient psychiatric patients.
• Provide leadership and psychosocial support to meet individual needs.
• Utilize knowledge of community and healthcare resources to address patient/client needs throughout their care journey.
• Ensure timely and appropriate interventions to enhance resource utilization and care quality.

Qualifications

• Master's degree in social work, psychology, or related field.
• Minimum of 3-5 years of relevant experience.
• Maryland Licensed LCSW required; LMSW candidates will also be considered.
