Job Description

We are looking for a passionate and experienced Social Worker to join our team at LifeBridge Health in Baltimore, MD. As a Social Worker-I/P Psych LMSW, you will play a crucial role in coordinating psychosocial assessments and discharge planning for inpatient psychiatric services at Sinai Hospital, providing essential support to meet individual needs throughout the care continuum.

Responsibilities

• Coordinate psychosocial assessment and discharge planning for inpatient psychiatric patients.
• Provide psychosocial support and resources to patients throughout their care journey.
• Utilize community and healthcare resources to address patient needs effectively.

Requirements

• Master's degree in social work, counseling, or related field.
• Previous experience in social work.
• Maryland Social Work licensure - LMSW required, LCSW-C preferred.
