Job Description

We are looking for a dedicated Social Worker-I/P Psych LMSW to join our team at LifeBridge Health in Baltimore, MD. In this full-time role, you will serve as a coordinator of psychosocial assessment and discharge planning for inpatient psychiatric patients, providing essential leadership and support to meet individual needs throughout the care continuum.

Responsibilities

• Coordinate psychosocial assessments and discharge planning for inpatient psychiatric patients.
• Provide leadership and psychosocial support to address individual needs.
• Utilize knowledge of community and healthcare resources for effective interventions.
• Ensure quality resource utilization and optimize length of stay and level of care.

Requirements

• Master's degree in social work, counseling, or related field.
• Licensed as an LMSW in Maryland, with preference for LCSW-C.
• Prior experience in social work is required.
