As a Care Manager, you will provide high-quality care and services to residents in our assisted living and reminiscence neighborhoods. Responsibilities

• Build relationships with residents and families, assist with activities of daily living, and attend to individual care needs.
• Monitor changes in residents' conditions and recommend appropriate care adjustments.
• Participate in the development of Individualized Service Plans and ensure residents' well-being during the shift.
• Collaborate with the community team to maintain safety standards and promote a welcoming environment.
• Support residents in life skills and enriching activities to enhance their quality of life.

Qualifications

• Positive attitude and flexibility in serving residents.
• Previous experience with seniors is preferred.
• Effective communication, organizational, and problem-solving skills.
• Ability to make decisions in the best interest of residents.
