Job Description

We are currently seeking an experienced Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning.

Responsibilities

• Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
• Implement care plans that are goal-oriented and patient-centered.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED required.
• Current DE LPN license or compact state license.
