Job Description

We are currently seeking experienced Home Care LPNs to join ChristianaCare's HomeHealth team. As an LPN with ChristianaCare, you will provide skilled nursing care to geriatric patients in the community, implement goal-oriented care plans, and contribute to discharge planning.

Responsibilities

• Provide skilled nursing care and treatment to geriatric patients.
• Implement patient-centered care plans in alignment with available resources.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED.
• Current DE LPN license or compact state license.
