Job Description
We are looking for an experienced Home Health Licensed Practical Nurse (LPN) to join our team at ChristianaCare. If you are passionate about providing skilled nursing care to geriatric patients in the community and implementing patient-centered care plans, this position is for you. You will have the opportunity to work full-time, part-time, or as per diem, supporting patients in New Castle County, DE.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
