Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a Home Care LPN, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.

Weekly Schedule
- Expected hours: 20-40 per week.
