Job Description
We are looking for a dedicated Home Care LPN to join our ChristianaCare HomeHealth team. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. We offer a supportive environment with state-of-the-art tools and comprehensive training.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
