Responsibilities
- Administer skilled nursing care and treatments to geriatric patients in the community.
- Develop and implement patient-centered care plans with a focus on goal achievement.
- Participate in discharge planning processes for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
