Job Description
We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. If you are a dedicated Licensed Practical Nurse seeking a supportive work environment and comprehensive training, we want you to be part of our team.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans and assist in discharge planning.
- Support patients in New Castle County, DE, with varying shift responsibilities and weekend/holiday rotations.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
