Job Description

We are looking for a dedicated Home Care LPN to join our team at ChristianaCare. As part of our HomeHealth team, you will provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans and participating in discharge planning.

Responsibilities

• Provide skilled nursing care and treatment to geriatric patients.
• Implement goal-oriented and patient-centered care plans.
• Participate in developing discharge plans for patients.

Requirements

• High School Diploma or GED.
• Current DE LPN license or compact state license.
