Job Description

We are looking for an experienced Home Care LPN to join ChristianaCare's HomeHealth team. In this role, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Key Responsibilities:

• Deliver skilled nursing care and treatments to patients.
• Implement goal-oriented and patient-centered care plans.
• Participate in developing patient discharge plans.

Requirements:

• High School Diploma or GED.
• Current DE LPN license or compact state license.

Job Types: Full-time, Part-time, Per diem

Pay Range: $22.00 - $28.00 per hour

Expected Hours: 20 - 40 per week
