Responsibilities

• Provide skilled nursing care and prescribed treatments to geriatric patients in the community.
• Implement patient-centered care plans consistent with available resources.
• Participate in discharge planning for patients.

Requirements

• High school diploma or GED required.
• Current DE LPN license, or compact state license, is a must.
