Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a member of our HomeHealth division, you will provide skilled nursing care to geriatric patients in the community, implementing care plans tailored to patient needs and goals. Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

Work Schedule
- Job Types: Full-time, Part-time, Per diem.
- Pay: $22.00 - $28.00 per hour.
- Expected Hours: 20 - 40 per week.
- Weekly Schedule: 5x8, Monday to Friday, rotating weekends.
- Work Location: In person.
