Job Description
ChristianaCare HomeHealth is seeking compassionate and experienced Home Care LPNs to join our dynamic team. With a focus on geriatrics, pediatrics, wound care, and home health services, our organization provides comprehensive support for our community. As an LPN, you will deliver skilled nursing care, implement care plans, and participate in discharge planning, ensuring the highest quality of care for our patients.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
Work Schedule
Expected hours: 20-40 per week. Day shift or night shift, one weekend a month, and 2 holidays a year.
