Job Description
We are looking for experienced Home Care LPNs to join our team at ChristianaCare. As a Home Care LPN, you will provide skilled nursing care to geriatric patients in the community, implementing patient-centered care plans and contributing to discharge planning. Join us to be part of a supportive team and a technologically advanced environment.
Responsibilities
- Provide skilled nursing care and prescribed treatment in the community.
- Implement patient-centered care plans and discharge planning.
- Support New Castle County, DE, with day or night shifts, one weekend a month, and 2 holidays per year.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
