Job Description
We are looking for experienced Home Care Licensed Practical Nurses (LPNs) to join our dedicated team at ChristianaCare HomeHealth. As an LPN with us, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement patient-centered care plans, and contribute to positive patient outcomes.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement goal-oriented, patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.

We offer competitive pay ranging from $22.00 to $28.00 per hour based on experience. The expected work hours range from 20 to 40 hours per week with opportunities for Per Diem, Part-Time, and Full-Time positions.
