Job Description
We are looking for an experienced Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse, you will be responsible for providing skilled nursing care and treatment to geriatric patients in the community. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must have a current DE LPN license or compact state license.
