Job Description
We are looking for an experienced Home Care LPN to join our team at ChristianaCare. As a Licensed Practical Nurse in our HomeHealth division, you will have the opportunity to work with a state-of-the-art Electronic Health Record system and receive comprehensive OASIS-E training. Our supportive leadership team is dedicated to helping you advance in your HomeHealth career.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED.
- Current DE LPN license or compact state license.
