Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join ChristianaCare HomeHealth team. As an LPN in our organization, you will have the opportunity to provide skilled nursing care to geriatric patients in the community, implement care plans, and participate in discharge planning. Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.
