ChristianaCare HomeHealth is seeking dedicated Licensed Practical Nurses (LPNs) to join our team. As an LPN in our Home Care program, you will provide skilled nursing care and treatment to geriatric patients in the community. This role involves implementing care plans, participating in discharge planning, and ensuring patient-centered care.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
