Job Description
We are looking for dedicated Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. Our state-of-the-art Electronic Health Record system and comprehensive OASIS-E training provide you with the tools to excel in your career. Join us in delivering skilled nursing care to geriatric patients in the community and play a vital role in developing patient-centered care plans.
Responsibilities
- Provide skilled nursing care and treatments to geriatric patients.
- Implement goal-oriented and patient-centered care plans.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or General Education Diploma (GED) required.
- Must hold a current DE LPN license or compact state license.
