Responsibilities
- Provide skilled nursing care and treatments to geriatric patients in the community.
- Implement patient-centered care plans in line with resources available.
- Participate in discharge planning for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
