Job Description

Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in the community.
- Implement patient-centered care plans with a focus on goal attainment.
- Participate in discharge planning for patients.
Requirements
- High school diploma or GED.
- Current DE LPN license or compact state license.
