Job Description
We are looking for a dedicated Licensed Practical Nurse (LPN) to join our Home Care team at ChristianaCare. As a valuable member of our organization, you will provide skilled nursing care to geriatric patients in the community, implement care plans, and contribute to discharge planning. This position offers the opportunity to work full-time, part-time, or per diem, supporting New Castle County, DE.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients in a community setting.
- Implement goal-oriented and patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must hold a current DE LPN license or compact state license.

Pay
$22.00 - $28.00 per hour
