Job Description
ChristianaCare is seeking dedicated Licensed Practical Nurses (LPNs) to join our HomeHealth team. This position offers the opportunity to provide skilled nursing care to geriatric patients in the community, ensuring patient-centered treatment plans are implemented effectively.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
