Job Description
Are you an experienced Licensed Practical Nurse (LPN) looking to join a supportive and well-equipped HomeHealth team? ChristianaCare is seeking dedicated LPNs to provide skilled nursing care to geriatric patients in the community. As part of our team, you will implement patient-centered care plans and contribute to discharge planning while benefiting from a comprehensive benefits package and ongoing training opportunities.
Responsibilities
- Provide skilled nursing care and prescribed treatments to geriatric patients in the community.
- Implement goal-oriented and patient-centered care plans in alignment with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED.
- Current DE LPN license or compact state license.

Compensation
Salary: $22.00 - $28.00 per hour
Expected Hours
20 – 40 per week
