Job Description: Licensed Practical Nurse MDS Coordinator

Genesis Health, a leading provider of healthcare services, is seeking a dedicated Licensed Practical Nurse (LPN) MDS Coordinator to join our team. In this role, you will play a crucial part in the Resident Assessment Process by completing the Minimum Data Set (MDS) and Care Area Assessments (CAAs) to ensure optimal patient care and nursing center reimbursement.

Position Highlights:

• Direct the interdisciplinary Resident Assessment Instrument (RAI) process, maintain compliance with guidelines, and oversee the completion and transmission of MDS.
• Develop individualized resident Care Plans, identify resident needs, and ensure compliance with Federal and State regulations.
• Train new team members on clinical content and electronic completion of MDS items.

Requirements:

• Current LPN license in good standing
• Prior experience in hospital, long-term care facility, or other healthcare setting
• Proficient in general nursing practices and procedures
• Knowledge of laws and regulations governing long-term care
• Computer skills including Windows applications
• AccuMed experience preferred
• Prior experience as an MDS Coordinator preferred
• Proficiency in RAI/PPS processes
• Current BLS/CPR certificate
