Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN in our organization, you will provide skilled nursing care and treatment to geriatric patients in the community, implement patient-centered care plans, and contribute to discharge planning. This position offers competitive pay based on experience and the opportunity to work full-time, part-time, or on a per diem basis.
Responsibilities
- Provide skilled nursing care and treatment to geriatric patients.
- Implement patient-centered care plans in line with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Current DE LPN license or compact state license.
