Job Description
We are looking for experienced Licensed Practical Nurses (LPNs) to join the ChristianaCare Home Health team. As an LPN, you will provide skilled nursing care and treatment to geriatric patients in the community, implementing patient-centered care plans and assisting in discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement patient-centered care plans consistent with available resources.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma or GED required.
- Must possess a current DE LPN license or compact state license.
