Job Description
We are looking for experienced and dedicated Licensed Practical Nurses (LPNs) to join our HomeHealth team at ChristianaCare. As an LPN, you will provide skilled nursing care to geriatric patients in the community, implementing care plans and participating in patient discharge planning.
Responsibilities
- Provide skilled nursing care and prescribed treatment to geriatric patients in the community.
- Implement goal-oriented, patient-centered care plans.
- Participate in developing discharge plans for patients.
Requirements
- High School Diploma (HSD) or GED required.
- Current DE LPN license or compact state license.
